I've been perusing the manual for LW6 and can't seem to find reference to this. I end up re-creating user field names. How do I keep them ?

There isn't any built-in way to do it, just take a blank show file and set the column definitions and names to whatever you want, then save the show file with a name like "My LW Template.lw6".
If you're on a Mac, you can right-click (ctrl+click) the template file and choose "Get Info", then check-mark the "Stationery pad" option. With that set, double-clicking the file automatically creates a brand-new file that's a copy of the original. This works on any kind of document, not just Lightwright files.
On Windows, the only thing that can help is to make the template file read-only, so you can't overwrite it.

The real shame is that doesn't work when you are receiving the LW file from the Designer to begin your layout. I end up rebuilding my fields every time because I'm never the person who originates the file.

You can always try merging the files, provided the designer didn't populate the User Fields you've renamed.

It's not about opening a new file from scratch. It's modifying a file that you have received from another user so that your custom user fields appear.

I definitely recommend having a template file and then merging in your designer's data. If the designer uses the same custom columns you can always move the data to an empty one.
I keep a template file for each venue I regularly ME in. That lets me be very detailed about the venue equipment, positions, etc and not have to do that set up every show.
I'm currently redoing mine for LW6 so that I can take advantage of the new features.
